ACCREDITATION PROCESS

Accreditation process cover the following basic steps :

Submission of application

Formal application is received. Application checked for basic information.

Application accepted.

Applicant invoiced and payment made.

Application ready to be formally processed.

Stage 1

Documentation review (also known as system assessment or adequacy audit)

A review of the applicant's documentation is undertaken against relevant standards

Stage 2

Pre-asessment (optional)

An applicant can request for a pre-assessment to be conducted

Stage 3

Compliance assessment (including witnessing of CB's auditors in action)

A compliance assessment carried out on the applicant's documented quality system, against STANDARDS MALAYSIA Accreditation Criteria. Stage 3 involves the witnessing of applicant's certification auditors in action.

Review of assessment reportAssessment report reviewed by an impartial and independent Accreditation Review Panel (ARP) who will then make a recommendation.
Approval of accreditation or other decisionsDirector General makes final decision on accreditation
Award of accreditationCertificate of accreditation is granted to successful applicant. Certificate is valid for 3 years.
Maintenance of accreditation (Surveillance assessments)Accredited CB will be subjected to surveillance assessments at every six (6) month intervals to ensure continuing compliance with STANDARDS MALAYSIA Accreditation of three years, the frequency of surveillance will be reduced as stipulated in ACB 7.
Re-assessment (including witnessing of CB's auditors in action)Certificate of accreditation expires at the end of third year. Prior to expiry, a comprehensive re-assessment is conducted on accredited CB's quality system.
Witnessing

Witnessing of certification audits is part and parcel of maintenance activity associated with each accredited programme. STANDARDS MALAYSIA may also request witnessing of audits for dormant and/or critical scopes.
