Application for registration of ACB assessor should be accompanied by: 1. Candidate's curriculum vitae (FM 501)
2. Confidential evaluation and summary of qualification of STANDARDS MALAYSIA assessors in quality system competence
3. Sector competence matrix form
4. Candidate signature attesting agreement to abide with STANDARDS MALAYSIA Code of Ethics for assessors
5. Training records
6. Completed letter of undertaking of maintaining confidentiality
